Dr. Domenico Ricucci received his degree in General Medicine from “La Sapienza” University of Rome in 1982, followed by his DDS in 1985. Since then, he has maintained private dental practices limited to endodontics. In addition to his private practice, Dr. Ricucci was Professor of Cariology at “Magna Graecia” University of Catanzaro, Italy in 2002 -2003 and served on the Research Committee of the European Society of Endodontology from 1999 to 2005. Since 1998, Dr. Ricucci has run his own histology laboratory and has developed considerable skills in hard tissue preparations for light microscopy. His research interests include: -Pulpal and periapical tissue reactions to caries and treatment procedures -Biofilms in endodontic infections -Etiology of root canal treatment failure -Pulpal regeneration/revascularization. Dr Ricucci has published 112 papers and lectured nationally and internationally. He has authored the Textbook and Atlas “Patologia e Clinica Endodontica” and “Endodontology. An integrated biological and clinical view”, which have been translated into Chinese, Russian, and Japanese. Furthermore, he has authored or co-authored 22 book chapters. Dr. Ricucci has no past proprietary or relevant financial relationship or receives gifts in kind (including soft intangible remuneration), consulting position or affiliation, or other personal interest of any nature or kind in any product, service, course and/or company, or in any firm beneficially associated therewith. There are no course prerequisites for this event.

Abstract: The apical limit of root canal instrumentation and obturation is a controversial issue in modern endodontics. Over the last several decades, the concept of moving small instruments through the apical constriction with the intention of preventing debris accumulation, canal obstruction, and disrupting bacterial biofilms, has gained increasing popularity among clinicians. However, the available literature often generates confusion and uncertainty for practitioners seeking clinically relevant information supported by evidence. The purpose of this presentation is to offer an evidence-based evaluation of apical patency within the context of apical anatomy, microbiology and histology. My research indicates that the best histologic response is observed when adequate instrumentation and homogeneous obturation are conducted to the apical constriction, without apical patency. With these findings in mind, we must also conclude that the “zero reading point” indicated by most apex locators corresponds to a point that is out of the canal, in the adjacent periodontal ligament. Clear philosophic differences regarding the apical limit and patency have developed over time. This research provides an objective evaluation of the apical limit and patency from which evidence based and clinically relevant conclusions may be drawn. Learning Objectives:
